VERY IMPORTANT IMFORMATION
As part of the registration process, students fill out health Forms. It is imperative students get the required immunizations before they register for classes. Students under 18 years must have their parents sign the forms and mail them to the Health Center.

• The Student Health Immunization Form: Fill in the dates of two Measles, Mumps and Rubella (MMR). If you are unable to obtain documentation re-immunization is required or verification of your immunity to Measles, Mumps and Rubella through a blood test (need a copy of your blood test results or MMR's).
  
  Tuberculosis (MANTOUX) test is only required of all Students within one year of Date of Admisssion to The Masters College. If the Tuberculosis Test is Positive, you must have a chest X-Ray within 2 years. We need a copy of the Tuberculosis test results or a copy of the chest x-ray report.

• The Student health History Form enables us to take care of your health needs and to know your medical insurance provider. If you have your own medical insurance, make a copy of your insurance card (front and backside) and mail, email or fax it to TMC Health Center (together with immunizations).
